A large number of Souths played in 4♠ doubled. The vast majority won the club lead and led a ♦ from hand. When declarer won the lead with ♣K, West always played ♦6, East overtook with ♦9 and returned a ♥, resulting in +590. When declarer elected to win the ♣A and played a ♦, West always played ♦8, East overtook with ♦9 and gave West a club ruff, resulting in -100. Is the play of the ♣K so fiendishly clever that it induces West to forget that South bid ♣s freely at the 3 level or that West led the suit into declarer's strength? Or that, holding ♠A, East could always win the first round of trumps and give partner a ruff in the unlikely event that the lead was a doubleton?

I am probably being very dim, but I don't understand how the Club ruff beats it. According to my count you just lose Spade Ace, one diamond and one club ruff.

West gets a second club ruff with its third spade when East wins the first trump lead from dummy.

I wouldn't call you very dim, at least the way it was explained. The key is TWO ♣ ruffs are needed - the first time on trick three, and the first time N-S leads a trump, East needs to rise with the Ace and lead that 3rd ♣ for the 2nd ruff and -1.
