In most cases you won’t have the flexibility to give signals, and if your partner thinks you are signalling, he’ll often misread the situation. As for Qx in trump, that’s not a holding from which one will lead very often. I did see a. WC make that lead in a big match, but it didn’t work out well

With holdings that include an honour your leads are more often forced by not wanting to give up a trick (for example leading low from JTx in case partner has singleton queen). I can't think of any recent hand where I led trump from Qx or QJx; from QJ tight I try to randomise a bit if I'm leading the suit.

In addition, on many hands it’s not possible to know whether you are helping declarer more than partner before seeing dummy. Plus some holdings simply make it extremely difficult for partner to work out at trick one what you may be trying to say. Finally, what if we have useful holdings in two suits. I’m not good enough to know which suit I want to tell partner (and declarer) what information will be most useful.
Btw, if you give ANY systemic information with your trump spot, and declarer asks ‘leads and carding’ (as I literally do every hand unless I’ve already been informed) you had better tell the declarer what your agreement is. Secret carding agreements are…well…you fill in the blank. And please don’t claim that suit preference (or count) is standard on opening lead…it isn’t.

1) when opponents have bid garbage stayman looking to ruff clubs
2) if opponents are 4-4 or 4-3 and you have 4 or more
3) if from the bidding you know partner is a bust and leading a side suit might finesse yourself
4) where the lead of a side suit might finesse your partner particularly where declare has shown strength
5 you hold a sequence like q-j-10 which aren’t particularly useful for trumping
6) when you have a longer minor trump suit than declarer
7) against a precision 3 suited opening 2D ususlly

And the relevant BOLS Bridge Tip is: "when leading trump, lead low." (with a comment from the editors that "when leading from equal or truly insignificant trumps, [choose] a card that shows suit preference". But I'd emphasize "truly insignificant" - the tip talks about the 7, from 742, being a key card with a particular trump layout).

However, when I see Peter play a 5 of trump, then a 7 and then out ( or7 then 5 then out) ., maybe we get one extra top board out of 100, but we will take it.
We will start to do the same with leafs of xx or xxx when a trump lead on the open seems right
