If I am reading this right...it is a double, followed by 1NT by the doubler. If, so, 18 is low. Many 18's should just overcall 1NT; it has a top of 20 in either case.

I play them as 19-21, whatever it is the NT range above your 1NT overcall.

The same.
For me this question goes in the category of "life's too short to make a significant difference" and the memory load of playing something different causes more trouble that it is worth.
